Caution: This calculator is for informational purposes only and should not be used to determine daily doses when switching a patient from one opioid to another or changing the route of administration, e.g., from IV to orally. This is especially important when switching a patient to or from fentanyl. Equianalgesic daily doses displayed are only approximations and do not account for genetic factors, incomplete cross-tolerance, and pharmacokinetics.

There are special considerations for calculating Methadone (Methadose, Dolophine) MED. This calculator uses the following conversion factors: for up to 20 mg of Methadone per day the conversion factor is 4, for 21-40 mg per day the conversion factor is 8, for 41-60 mg per day the conversion factor is 10, and for more than 60 mg per day the conversion factor is 12.
